NEW YORK, April 18, 2013 /PRNewswire/ — Centers Plan for Healthy Living, a new Medicaid Managed Long Term Care Plan started enrollment activities February 1, 2013. The Plan’s goal is to coordinate care to Medicaid recipients over 21 years old with chronic needs in the New York City metropolitan area and Rockland, Niagara and Erie counties. The Plan, which can be found online at www.centersplan.com, is committed to involving the member, the member’s health care decision makers, their primary care doctor and any other involved professionals to optimize and coordinate the member’s plan of care.
“Our main goal in creating the Centers Plan for Healthy Living is to create a Plan which benefits not only our members, but their families, healthcare decision makers, and general caregivers as well. We would like our managed long term care plan to foster a collaborative and helpful community that ensures every effort is taken to provide our members with the most appropriate healthcare available. Our Plan received its license and Certificate of Authority from New York State in less than a year, and we simply can’t wait to bring our services to you and your family. With more than 19 covered services, we’re certain we can accommodate most individuals,” said Barry Volin, CEO of the Centers Plan for Healthy Living.
Managed long term care plans (MLTCP) are designed for people who are chronically ill or have disabilities which require more than 120 days of community-based long term care services. The Centers Plan helps its members and their families contact the medical providers that are right for them, both in coverage and location. The MLTCP also schedules and pays for a large selection of health and social services, including transportation to and from appointments, home health aides, social day programs, meals on wheels as well as many other benefits. A complete list of benefits can be viewed at the Plan’s website, www.centersplan.com. Prospective members must live in one of the NYC five boroughs or Rockland, Niagara and Erie to qualify for membership in the Centers Plan.
To become a member of Centers Plan for Healthy Living, you can call toll free (855-270-1600) during regular business hours with any questions or concerns. Once all questions have been answered, a nurse will make a home visit to determine needs, explain the mechanics of the program, and how it can be most beneficial to members. Once eligibility has been determined and an application to the state has been approved, membership will become effective on the first day of the subsequent month.
